Information & Rights

Care Navigators – Staff linked to Brent voluntary and community organisations who support care planned patients accessing relevant information and support. The Care Navigators work alongside GPs, social care and other provider organisations to advise and direct patients to relevant support and services.

Services Care Navigator Provide:

Introducing self-care and self-management to patients

Providing information and advice

Signposting to the right services

Improving access to information for patients

Support to develop manageable goals for patients

Break down barriers between different providers, professions and services

Aid decision making for patients

Referral Process

The Referral Process for Care Navigation is carried out through the GP for patients with a Care Plan. Your GP will let you know if this service is suitable for you.
